Definitely charge per invoice. Under the 2013 amended regulations you can also charge all your reasonable collection costs as well.

Regards the second point your should always bring the claim against the person (company) named in the contract. This is easy if your have a written contract, or the person completed account opening forms. If the client was an individual trading under a trading name its best to put both eg. 'Joe Blogs T\A xyz'. If in doubt bing the claim against the person you have dealt with and let them defend otherwise.
